Autumn Hills Assisted Living offers assistance with all activities of daily living, domestic services and nutritious meals. Our lovely home can improve the quality of life for our residents by increasing socialization, companionship, safety, and  meeting medical needs of each individual  resident. 

We are staffed 24 hours a day to provide your loved one with all of the personal care, supervision and security they may need.

Our Assisted Living facility is able to provide a high level of care and increased personal satisfaction at a reasonable cost to the consumer. Autumn Hills is licensed by the Minnesota Department of Health.

Autumn Hills Provides:

  • Home-like living with the assistance to meet the special needs of the elderly or disabled
  • Companionship and socialization through scheduled activities.

    •  awake staff 24 hours per day
  • Personal services including bathing, dressing, grooming, toileting, walking, wheeling, etc.
  • 3 nutritious meals per day plus snacks

  • Daily cost is based on an individuals care needs as determined by the RN assessment
  • Pay sources accepted include: private, elderly waiver,  CADI, and long-term care insurance.

About the Facility

Autumn Hills is entirely handicapped accessible.
There are Three Wings
The North East Wing has 10 single apartments all with private bathrooms and walk-in showers.

The South East Wing has 9  single apartments all with private bathrooms and walk-in showers.

The West Wing has 11 single apartments and 1 double apartment.  3 singles and the double room each have a private bathroom with walk-in shower.  The other 7 apartments share four bathrooms.  These bathrooms have large handicap accessible showers and a handicap walk-in bathtub. 

3 large common areas
North one has comfortable seating, TV, Fireplace and piano. Central one has comfortable seating and a birdcage with 4 parakeets. West one has comfortable seating and a large television.

Dining
Our spacious dining room has lots of windows to let the sun in. There is an ice/water machine, coffee machine and a pop machine available 24 hours per day for staff and residents.

Owners & Staff
Autumn Hills was opened in 2005. It is owned by a group of community and service-oriented individuals. The facility is professionally managed by experienced staff.

Autumn Hills is staffed with caring, dependable Nurses and Home Health Aides who are trained in all aspects of Home Care.

All employees are required to pass a criminal background check and complete intensive orientation training along with  annual continuing education.

Other Amenities and Activities



We have a sunny courtyard for your enjoyment with lots of seating. Raised gardens for vegetables and an assortment of flowers planted by the residents and staff. There is a cement walking path that extends half way around the building.

In the summer months we participate in senior fishing trips on the pontoon, cookouts and relaxing on the porch.
​Activities are scheduled to provide our residents with mental stimulation and exercise.

Some other Activities Include:
Bingo, Bowling on the Wii, Craft Projects, Holiday                  Decorating, Baking, Painting, Gardening, Puzzles, Holiday Parties with Families, Musical Programs, Exercises, "Reminiscing" & "Senior Moments" games
